How do corticosteroids work in asthma?

چکیده

Corticosteroids (or glucocorticosteroids) are widely used to treat various inflammatory and immune diseases. The most common use of corticosteroids today is in the treatment of asthma, and inhaled corticosteroids have become established as first-line treatment in adults and children with persistent asthma, the most common chronic inflammatory disease. Recent developments in understanding the fundamental mechanisms of gene transcription (see Glossary) have led to major advances in understanding the molecular mechanisms by which corticosteroids suppress inflammation. This may have important clinical implications, as it will lead to a better understanding of the inflammatory mechanisms of many diseases and may signal the future development of new anti-inflammatory treatments. The new understanding of these new molecular mechanisms also helps explain how corticosteroids switch off multiple inflammatory pathways; in addition, it provides insights into why corticosteroids fail to work in patients with steroid-resistant asthma and in patients with chronic obstructive pulmonary disease (COPD).

Spirometry Findings Following Treatment with Oral and Inhalant Corticosteroids in Mild to Moderate Asthma Exacerbation in Children

Introduction:  Asthma exacerbation is common in children. Treatment with oral corticosteroids (OCS) and inhaled corticosteroids are suggested for asthma exacerbation. It is shown that inhaled corticosteroids has similar outcome in reducing asthma symptoms compared to OCS. But few studies have evaluated the pulmonary function changes in these two treatments. In this study, we evaluated the chang...

آینده درمان دارویی آستم

ABSTRACT: Asthma is a chronic disease in which hyperactivity of airways to different stimuli is responsible for pathophysiologic changes seen. With respect to the pathophysiologic changes seen the treatment can be divided into two groups: bronchodilator and anti-inflammatory drugs. Bronchodilators consist of beta 2 agonists (salbutmol), Xanthine (theophilline) and anticholinergic(ipratropium)...

Management of the patient with eosinophilic asthma: a new era begins

Now that it is generally accepted that asthma is a heterogeneous condition, phenotyping of asthma patients has become a mandatory part of the diagnostic workup of all patients who do not respond satisfactorily to standard therapy with inhaled corticosteroids. Late-onset eosinophilic asthma is currently one of the most well-defined asthma phenotypes and seems to have a different underlying patho...
